The LVN assists the RN by identifying patient physical and environmental needs, participating in care planning, and providing ordered physician services. Responsibilities also include patient, family, and caregiver education, effective communication with the team, and completing documentation within 24-48 hours.
Candidates must be a Licensed Vocational Nurse (LVN) and home health experience is required, although experience with Homecare Homebase EMR is a plus. The role involves assisting the RN in identifying and implementing patient care based on physician orders.
Under the direction of the RN, assists in identifying the patient’s physical and environmental needs as evidenced by documentation. Participates in planning and implementing care in partnership with the patient, family, caregiver, and the RN. Provides services that are ordered by the physician as indicated in the plan of care. Provides patient, family, and caregiver education. Provides effective communication to the patient, family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation within 24-48 hours.
