The Hospice RN Case Manager is responsible for providing professional nursing care to hospice clients, including assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes as prescribed by the physician. This role involves managing patient symptoms, assisting with grief/loss issues, coordinating care with the interdisciplinary team, and providing education to patients and caregivers regarding disease process and end-of-life care.
Candidates must be graduates of an approved Nursing education program and hold current Registered Nurse licensure in the state of practice, requiring a minimum of one year of experience as an acute care nurse, with home healthcare/hospice experience being preferred. Essential requirements include strong communication and interpersonal skills, especially in grief/loss management, along with a valid driver's license and reliable transportation.
