A bachelor's degree in a human services field is required. The role is entry-level and requires the ability to function as part of a multi-disciplinary team.

We are currently seeking a Full Time Case Manager for our Outpatient Department in Jersey City, NJ. Our Outpatient Program uses a multi-disciplinary approach; emphasis is placed on individualized assessment, goal-oriented treatment planning, periodic case review, and continuity of care for all persons served.
