Deliver compassionate skilled nursing care and treatments in clients' homes according to physician-directed care plans. Coordinate with RN Case Managers and educate families on home safety and infection control.
Must be a Licensed Practical Nurse (LPN) in Iowa with a valid driver's license and reliable transportation. One to two years of experience in acute hospital or home health care is preferred.

Conduct home visits and shifts, assessing client conditions, recording vitals, and noting any changes. Administer nursing procedures, manage medications, and provide treatments following physician orders and scope of practice. Promote a safe environment by educating families on home safety, infection control, and fall prevention. Maintain clear communication with the RN Case Manager and other team members to coordinate and enhance care quality. Document all care delivery accurately and in a timely manner.
