About the role

Assist physicians in the clinical care of patients through assessment, planning, and evaluation, specifically focusing on wound care. Support day-to-day clinical operations including data entry, scheduling, and collaborating on multidisciplinary care plans.

Responsible for assisting the physician in clinical care of all patients to include patient assessment, planning, intervention and evaluation. Will also have general responsibilities to assist in the day-to-day clinical operations of the office.

Education See License/Certification Requirements

Experience Two years of related experience. Requires working knowledge of specialized practices, equipment, and procedures.

Licenses & Certifications

*Georgia Licensed Practical Nurse or Multistate Nursing License *Healthcare Provider (CPR)

Qualifications

*Experience in medical practice preferred. *Must be capable of dealing tactfully and effectively with patients, family members, other employees and physicians. *Must maintain high degree of confidentiality. *Must possess the cognitive ability to understand patient's medical records, physician's orders and medical orders to be communicated to patient, hospital and/or other physician. *Must possess the sense of discipline to work in accordance with accepted clinical standards.

Qualifications

Area of Responsibilities *Assess compliance to the Clinical Pathway and communicates to Physician and rest of Clinical team. *Assists the Office Coordinator with various functions including data entry, scheduling of patients, and printing of information *Collaborates with Physician and Care team to develop a Multidisciplinary plan of care for the patient. Revises plan as needed with consultation with physician and clinical manager *Performs nursing assessment of patient's condition and reports findings to Case Managers and Clinical Manager *Records Wound Assessment data accurately and communicates pertinent information

Compliance Statement Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

Supervision *The LPN has limited supervisory duties but may instruct new employees, nursing technicians and correct work routines according to hospital policy and procedure.

Mental Demands Work involves a variety of problems in a general field, some of which are complex. Involves some independent judgment to decide what to do to assemble facts, determine variations from standard procedures, or plan other action to be taken to meet general objectives.

Physical Effort Moderate physical effort - Lifts, carries, or handles lightweight (1 to 25 lbs.) materials or equipment for about half of the day. Very occasional physical effort with medium weight objects (25- 60 lbs.). Office or laboratory work requires close visual effort and concentration more than half of day. Works in reaching or strained positions for less than half of day.

Working Conditions Considerable - (About 75% of the day) Involved in exposure to dirt, odors, noise, or some work is performed with exposure to temperature/weather extremes/occupational risk and probability of coming into contact with blood borne pathogens, other potentially infectious diseases, or biomedical/bio-hazardous materials.
