The LVN provides nursing care to hospice patients in their residences and coordinates with an interdisciplinary team to implement care plans. Responsibilities include performing on-call services, documenting patient needs, and communicating with physicians and families.
Candidates must be a graduate of an accredited nursing school with a current LVN state license and a valid driver's license. A minimum of two years of clinical nursing experience and one year of full-time hospice experience is required.
The Licensed Vocational/Practical Nurse performs nursing care for patients of all ages in their place of residence, coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency; and contributes to the coordination of care. The Licensed Vocational/Practical Nurse reports to the Hospice Administrator.
Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation clinical record, team report, and evaluations. Participates in planning and implementing care in conjunction with the RN, delivers and documents care based on the Interdisciplinary plan of care Provides nursing care utilizing infection control measures (OSHA) Assures the continuity of care through delivery of quality patient care. Work with the RN and the Interdisciplinary Group/Team to ensure patient/family needs are met. Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care. Timely submission of all required paperwork. Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency. Participates in the Hospice’s QAPI program. Promotes the Hospice’s philosophy and administrative policies. Meets productivity standards. Performs on-call responsibilities and provides on-call services to patients and their families as assigned. Provides effective communication to patients, their family members, team members, and other health care professionals.
Transportation: Reliable transportation and valid auto liability insurance
Education and experience: Two (2) years experience as a Licensed Vocational/Practical Nurse in a clinical care setting and one (1) year full time hospice.
Graduate of an accredited Diploma or Associate School of Nursing.
Licensure: Current State license as a Licensed Vocational/Practical Nurse; Current driver’s license
Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills
