Responsibilities

Essential Functions: Deliver exceptional care to the dedicated client in their home Help execute comprehensive personalized weekly schedules and care plans Provide skilled nursing services as prescripted under a client's plan of care that can be safely provided in the home, within your scope of practice Counsel clients and family in meeting nursing and related needs Meet all documentation requirements according to agency policy and proecedure Coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated Administrative functions such as documentation of completion of tasks and report changes in the client’s physical condition, mental capability or behavior

Qualifications
