Provide admission services, initial physical assessments, and care planning for potential hospice patients and their families. Coordinate initial medication and equipment needs while maintaining a compassionate and respectful environment.
Requires a graduate degree from an accredited school of nursing and a valid nursing license in the state of practice. Candidates must have at least 2 years of RN experience in a hospital or care facility setting and a valid driver's license.
9:00 am - 7:00 pm.
Part-time, 20 hours weekly.

Provision of the initial assessment and physical assessment for all patients being admitted to St. Elizabeth Hospice. Identifies and coordinates initial medication and equipment needs and completes the initial plan of care for all admissions. Demonstrate respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors, and other employees regardless of cultural background. MINIMUM REQUIREMENTS Education, Credentials, Licenses: Graduate of an accredited school of nursing. If eligible, must have a Compact (multi-state) License in their primary state of residence. Licensed to practice nursing in the state where work is being performed. Meets contact hour requirements for licensure, including all state-required courses. Valid Driver’s license. Specialized Knowledge: Demonstrates advanced assessment skills appropriate for home care environment. Ability to function independently. Knowledge of community resources.
