Conduct in-home client assessments and reassessments while maintaining accurate nursing documentation. Collaborate with the care team and caregivers to ensure high-quality care plans and continuity of support for clients.
An active RN license is required, with a preference for experience in home health, hospice, or working with Medicaid clients. Candidates must be tech-savvy, self-motivated, and possess reliable transportation for local visits.

This flexible PRN role is ideal for an RN looking for supplemental hours while making a meaningful impact in the community. The nurse will complete in-home visits, client assessments, and reassessments while collaborating with our care team to ensure exceptional client support and continuity of care.
Conduct in-home client assessments and reassessments Complete nursing visits and required documentation accurately and timely Observe and report changes in client condition or care needs Collaborate with office staff and caregivers to support quality care plans Provide compassionate, professional communication with clients and families Assist with additional nursing support as needed
PRN / Part-Time Estimated 2-12 hours per week with opportunity to grow Flexible scheduling based on client needs and availability
Active RN license required Previous home health, hospice, or healthcare experience preferred Experience working with Medicaid clients is a plus Strong bedside manner and communication skills Tech-savvy and comfortable using digital documentation systems Self-motivated with a flexible, “go-getter” attitude Reliable transportation for local visits
