The Clinical Care Partner coordinates patient care appointments and manages follow-ups for recently hospitalized or high-risk patients. They follow population health management standards and quality measures under the supervision of clinic and quality managers.
Candidates must be a graduate of an accredited medical assistant program or have equivalent experience, with at least 3 years of experience as a medical assistant. A BLS Certification is required.
