Position Summary

As a Home Health Physical Therapist, you will provide one-on-one care in patients' homes, helping them improve mobility, regain strength, and achieve greater independence while making a meaningful difference in their lives. Responsibilities Evaluate patients and develop individualized treatment plans. Provide therapy services to improve strength, mobility, balance, and function. Educate patients and caregivers on exercises and safety. Document visits and patient progress in the EMR. Collaborate with physicians and the interdisciplinary care team. Why Join Care2You? Qualifications Graduate of an accredited Physical Therapy program Current Physical Therapist license in the State of Michigan Current CPR certification Valid driver's license, reliable transportation, and auto insurance Minimum of one (1) year of Physical Therapy experience preferred Home health experience preferred, but not required Ability to work independently and manage time effectively Strong communication and interpersonal skills Proficient in electronic medical records (EMR) and documentation
