Deliver holistic, patient-centered care by assessing, planning, and implementing individualized care plans within a Relationship-Based Care framework. Collaborate with multidisciplinary teams to ensure optimal patient outcomes and maintain comprehensive clinical documentation.
Requires a current Registered Professional Nurse license and CPR certification based on AHA standards. BSN and experience with ACLS, Telemetry, and IV training are highly preferred.

Utilize critical thinking and strong communication skills to effectively convey patient conditions, collaborate with the care team, and recommend care plan adjustments. Ensure accurate, timely, and comprehensive documentation to support continuity of care and optimal patient outcomes. Provide education to patients, families, and caregivers using evidence-based teaching strategies tailored to individual learning needs. Guide patients through their care experience and support smooth transitions across care settings. Contribute to the development of students, new staff, and team members, including serving as a preceptor as appropriate, while fostering an environment of open communication, feedback, and continuous learning. Demonstrate ongoing commitment to professional growth and clinical excellence by participating in shared governance, unit initiatives, and organizational goals. Actively support change initiatives, adapt to evolving clinical practices, and contribute as an engaged and collaborative member of the care team. Participate in quality improvement and patient safety initiatives by identifying opportunities for enhancement, supporting evidence-based practice, and contributing to multidisciplinary discussions. Take an active role in improving patient satisfaction and care outcomes through the application of research, clinical expertise, and continuous improvement efforts. *Performs in accordance with system-wide competencies/behaviors. *Performs other duties as assigned.
