The LVN provides high-quality nursing care and implements individualized care plans for residents. Responsibilities include monitoring health conditions, administering medications, and collaborating with healthcare teams to ensure resident well-being.
Candidates must be a graduate of an accredited LVN/LPN program and hold a current Texas LVN license. A minimum of one year of nursing experience is required, with a preference for backgrounds in geriatric and dementia care.
LVN, 2pm-10pm
The Licensed Vocational Nurse (LVN) delivers high-quality nursing care to residents within the community, ensuring services are provided in accordance with established clinical standards and practices. Working under the supervision of a licensed nurse or healthcare manager, this role is responsible for implementing resident care plans safely and effectively. The LVN supports residents’ overall well-being by providing compassionate care, monitoring health conditions, and contributing to a positive, safe, and supportive living environment.
Resident Assessment: Evaluate residents’ physical, psychological, environmental, and social needs, identifying potential health concerns. Care Delivery: Assist residents with activities of daily living (ADLs), personal hygiene, and mobility while ensuring safe and high-quality care. Health Monitoring: Collect and analyze health information, maintaining accurate and up-to-date resident records. Treatment Implementation: Administer medications and treatments per physician orders, monitor for adverse reactions, and report concerns promptly.
Education: Graduate of an accredited LVN or LPN program.
Current Licensed Vocational Nurse (LVN) license in the state of Texas.
