Essential job functions
Position Specific
Provides comprehensive, strength-based, culturally competent, flexible, effective, quality substance use prevention and treatment services to eligible at-risk children, youth, and families
Provides services to assigned youth (ages 12-20) and their families/caregivers in the home, office, school, or other community based settings. May utilize telehealth per current agency guidelines.
Able to provide or arrange for safe transportation for youth and families to services as needed
Possesses specific cultural competence to work with and outreach to culturally diverse populations and communities
Ensures all clinical records are current and accurate, meeting organization and County timelines for submission
Provides and documents all Drug Medi-Cal eligible services provided, including but not limited to assessment, plan development, safety planning, individual therapy, family therapy, group sessions, case management, crisis intervention, and relapse prevention
Completes Screenings, Intake, Assessment, and Discharge packets for assigned youth, meeting expected timelines
Accurately documents all billable and non-billable services provided, meeting expected timelines
Provides referrals and resources to children, adults and families, and acts as a liaison between youth (ages 12-20) and their family/caregiver, assigned treatment team and external organizations
Utilizes identified substance use prevention and treatment models.
Ensures every assigned youth has an individualized safety plan that maximizes the use of natural supports and community based resources
Participates in the agency 24 hour family support on-call rotation
Participates in utilization, accreditation, peer review process as assigned

Coordinates with other services provided under the Drug Medi-Cal model, acting in collaboration with other partners and ensuring documentation reflects standards that meet or surpass the agency, state and federal Drug Medi-Cal/Medicaid standards

The posted salary bands represent the full pay range for each role and reflect opportunities for compensation growth over time. The typical hiring ranges reflect where we reasonably expect to make offers to new hires based on relevant experience, qualifications, and internal equity.
Level I • Typical hiring range
$75,000 – $86,500 per year
Full salary band: $75,000 – $100,000 per year
Level II • Typical hiring range
$80,000 – $92,000 per year
Full salary band: $80,000 – $105,000 per year
