• The person in this role is concerned with continuity of care which usually involves discharge planning for children who are inpatients and their families to ensure they understand and follow the recommended treatment plan, and referrals to community agencies and programs.

• Experience: 1+ years of previous social work or related field experience required

• Functions: Understands and satisfies the needs of the neonatal, infant, toddler, pre-school, school-age, adolescent and/or adult patient in regards to their growth and development process.
• Assists families in meeting their needs through: Short term, crisis oriented counseling; referrals to community agencies, programs and services; educating patients/families about illness, hospital procedures and community resources. Helps plan for the future health care needs of the child whether or not admitted to the hospital. Consults with other professional staff to assure that they understand the strengths and needs of patients and families. Assesses the medical care/history of the child and family to assist medical staff with diagnosis and treatment plan. Performs duties in courteous and helpful manner with respect towards patients/families, department staff, visitors and employees from other departments.
