A Home Health Aide / Hospice Aide is a certified health care worker who assists professional personnel in the personal cares of the patient by providing necessary and desired services. The duties of the Aide are planned and assigned by a professional member of the health care team to meet the needs of the patient and family situation.

Duties & Responsibilities
Provides a sense of security for the patient and performs personal cares while respecting the dignity, comfort and privacy of the patient at all times. Provides physical and emotional support to the patient and family unit.
Adapts to changes in the scheduling process to meet the needs of the patient.
Accepts direction in a positive manner, eliminates assumptions, exhibits effective listening skills and follows through with instructions given.
Adheres to strict patient confidentiality standards as mandated by HIPAA.
Utilizes supplies in a safe, proficient, effective and orderly fashion.
Documents tasks performed accurately and in a timely manner in the patients’ clinical record. Offers ideas for and participates in Continuous Process Improvement activities. Demonstrates participation in continuing education provided and attends staff meetings as scheduled.
Essential Skills and Experience

Internal Applicant Policy
It is the policy of Winona Health to work with employees in an equitable fashion regarding promotions, transfers, and position reclassifications.
Any employee wishing to apply for another available position at Winona Health must submit an application for consideration through the internal application portal.
Employees requesting to change positions should have successfully completed at least six (6) months in their current position.
