This position provides discharge planning assistance, supportive counseling, and advocacy services to assigned patients and their families.
Assesses patient's social services and discharge planning needs and their ability to provide for those needs post discharge. Identifies situations indicating possible abuse, dependencies, growth and development problems and follows up appropriately. Provides support to patients and families coping with illness and/or hospitalization. Follows regulations while working with patients and/or families for post-acute placement. Makes recommendations regarding needs of patients and suggestions for providing more effective comprehensive care and social services. Refers patients to community or other appropriate services post discharge. Reports possible abuse cases to appropriate community agencies for further investigation. Documents all assessments, referrals, and interventions in the patient’s medical record. Maintains professional relationships with post-acute facilities for referrals and placements. Attends multi-disciplinary rounds to support length of stay goals. Assesses patients when requested by nursing, patient/family and/or physician. Works with nursing, ancillary and medical staff with high degree of professionalism and tact. Attends and/or facilitates family meetings when requested.
Master's degree in Social Work or in the related fields of Guidance and Counseling, Human Services, Marriage and Family Counseling, Psychology, Sociology, and Vocational/Disability Rehabilitation, from an accredited college or university required. Internship required. Previous experience in medical social work or post-acute facility preferred. NJ Licensed Social Worker or Licensed Certified Social Worker required. Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team. CSHS considers many factors to determine compensation, including education, experience, skills, licenses, certification, and training. As such, team member compensation may fall outside this range.
