Coordinates the continuum of care for adult oncology and hematology patients from referral through treatment. Manages daily clinic activities and implements research protocols in collaboration with a multidisciplinary team.
Requires a Bachelor of Science in Nursing, a current California RN license, and BLS certification. Minimum of one year of nursing experience is required, with oncology and case management experience preferred.
The Patient Navigator-RN Cancer is a skilled oncology nurse and specializes in caring for patients eighteen years of age or older with suspected or confirmed malignancies or a hematologic disorder in the outpatient Cancer Clinic. Utilizes discretion and independent judgment in applying the nursing process to the provision of age-specific and developmentally appropriate quality patient care. Case manages and coordinates the continuum of care from referral through the treatment process in collaboration with the physician and the multidisciplinary team. Proactively manages daily clinic activities to ensure efficiency and timely patient visits. Implements research protocols in collaboration with the research team. Facilitates the empowerment of the nursing profession and embraces new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Performs other duties as needed.
Bachelor of Science Degree in Nursing required. Minimum one year of nursing experience required. Minimum one year of oncology nursing preferred. One year case management or transitional care management experience preferred.
Current California Registered Nurse (RN) license required. Basic Life Support certification issued by the American Heart Association required. Oncology Certification Nurse (OCN) preferred.
