Requirements: and Responsibilities:
Evaluates and provides initial treatment with patients recovering from injury, disease, or surgery in a home care environment.
Establishes treatment plans using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.
Works cooperatively with physicians, case managers and treating therapists.
Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
Requires state license to practice physical therapy.
Reports to manager of clinical services.
