Provide compassionate personal care and assistance with activities of daily living to patients in their homes. Monitor patient conditions, document information in the EMR system, and collaborate with the care team to implement plans of care.
Requires a high school diploma, a Home Health Aide or similar certification, and at least one year of experience as an HHA or CNA. Must possess an active CPR certification, a valid driver's license, and reliable transportation.
Supports a patient’s Plan of Care (POC) by monitoring and recording patient condition, providing support and personal services, and teaching families Monitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise Performs patient assessments and collaborates with the multi-disciplinary care team to develop and implement a plan of care Contributes to the continuity of care through effective communication and partnership with the designated Care Team Supports patients by providing housekeeping or laundry services, shopping, other household tasks, and preparing and serving meals and snacks Provides personal care services including bathing, bed baths, shower assistance, dressing, grooming, shampooing, oral hygiene, shaving, skin care, incontinent care, and assistance with toileting Assists patients with safe transfers, ambulation, repositioning, and range-of-motion exercises as directed by the care plan Helps family members care for their loved one by teaching appropriate ways to lift, turn, reposition, and safely assist the patient Interfaces with family members to address concerns, answer questions, and educate them on the patient’s treatment and aftercare needs Records patient information via our EMR system and notifies branch leadership of any changes or unusual conditions Enhances service reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to our accomplishments Performs on-call responsibilities and on-call services to patients/families if assigned Maintains a safe, secure, and healthy environment by following asepsis and universal precaution controls in performing client care Protects the agency by adhering to professional standards, home care policies and procedures, and federal, state, and local requirements Maintains mandatory continuing education requirements (CEUs) of the Agency/licensing board
