$51,830 - $79,300 / YEAR
Provides direct nursing care and assessments to patients in their homes based on physician-ordered treatment plans. Collaborates with RNs and MDs to report changes in patient condition and implements health care teaching plans for patients and caregivers.
Requires graduation from an accredited practical nursing school and a current New York State license to practice as a Practical Nurse. Prior acute care experience is strongly preferred.
Performs assessments, nursing procedures, within the framework of the plan of care established by the physician.
