As a Case Manager / Group Facilitator, you will be part of that impact — helping individuals move from crisis to stability and from isolation to connection. This role combines group facilitation and case management, allowing you to build strong connections with clients while supporting their overall treatment journey. You will plan and lead groups, support individual client needs, and collaborate with a multidisciplinary team to ensure clients are progressing and receiving the appropriate level of care. This position is ideal for someone who is engaging, organized, and passionate about helping others grow in a structured, supportive environment.

High School Diploma with Substance Use Disorder (SUD) Registration Certification OR Bachelor’s Degree in Psychology, Social Work, or related field Minimum one (1) year of experience facilitating behavioral health or mental health groups Valid Driver’s License with clean driving record
