The Physical Therapist evaluates residents to develop and implement individualized treatment plans focused on mobility, strength, and functional independence. They collaborate with interdisciplinary teams and educate residents and caregivers on safety and exercise.
Candidates must hold a Doctorate or Master's Degree in Physical Therapy and a current California Physical Therapist license. Experience with Casamba and PointClickCare is preferred.

Essential Duties Evaluate residents and determine physical therapy needs and rehabilitation potential Develop, implement, and modify individualized treatment plans Provide treatment focused on gait training, transfers, balance, strengthening, mobility, and pain management Communicate with physicians, nursing staff, residents, and families regarding progress and goals Participate in care plan meetings and interdisciplinary team conferences Complete timely, accurate, and compliant documentation Educate residents and caregivers on exercises, mobility safety, and fall prevention Recommend assistive devices and durable medical equipment such as walkers, canes, and wheelchairs Supervise PTAs and therapy aides as appropriate Maintain a safe treatment environment and follow infection control procedures Qualifications Education: Doctorate or Master’s Degree in Physical Therapy from an accredited program required License: Current California Physical Therapist license in good standing Preferred Skills: Experience with Casamba and PointClickCare Strong communication and interpersonal skills Excellent clinical judgment and problem-solving abilities Passion for patient-centered care and functional outcomes We are an equal opportunity employer.
