Must be a graduate of a state-approved Physical Therapist Assistant program with a current New Hampshire license. Requires a valid driver's license, access to a vehicle, and the ability to use portable electronic devices for clinical documentation.

Implement patient care plans developed and supervised by a Registered Physical Therapist, delivering hands-on therapeutic services in patients' homes and community settings. Provide skilled therapeutic interventions to maintain, improve, or restore patient function across neuromusculoskeletal, pulmonary, and cardiovascular conditions. Collaborate with the care team including supervising therapists, nurses, physicians, and other providers to ensure coordinated, patient-centered care. Educate patients and families on therapeutic procedures, home exercise programs, and strategies to support recovery and independence. Document clinical progress accurately and in accordance with HCS standards and regulatory requirements. Monitor and ensure patient safety during all therapy sessions, relaying significant changes in patient status to the supervising PT in a timely manner. Why Join Us? Graduate of a state-approved Physical Therapist Assistant program One year of experience under appropriate supervision; or a combination of education and experience demonstrating the necessary competencies for the position Current New Hampshire license to practice as a Physical Therapist Assistant Ability to utilize a portable electronic field device and manage ePHI remotely in accordance with HCS policies Valid driver's license and access to a vehicle meeting NH registration and auto liability insurance requirements
