Inova Transitional Care (ITC) offers a unique and rewarding opportunity to make a meaningful impact on patients at one of the most critical points in their healthcare journey—transitioning safely from hospital to home.
Our program is dedicated to delivering high-quality, patient-centered care to high-risk adult populations, including uninsured and underserved patients, who require timely follow-up after discharge from an Inova hospital or emergency department.
As part of a collaborative, mission-driven team, you will help bridge gaps in care for patients who may not yet have access to a primary care provider.

This role offers the opportunity to build meaningful patient relationships while contributing to both clinical care and support services, including connecting patients to community resources and helping address social drivers of health.
This position is primarily based in our Fairfax clinic, with flexibility to support our Alexandria location—offering exposure to diverse patient populations and care settings.
