Provide individualized guidance and restorative support to adults with serious mental illnesses to assist them in maintaining community living. Develop self-management plans and Individualized Action Plans while helping consumers integrate into their communities and reduce stigma.
Requires a high school diploma or GED and a valid driver's license with a reliable vehicle. Candidates must have lived experience with mental health services and the self-knowledge to manage their own recovery process.
Assist consumers with being able to identify personal methods of recovery from mental illness and to develop self management plans to address their experiences. Participate in the development of Individualized Action Plans (IAPs) and assist consumers with developing a self-management plan. Assist consumers in being fully integrated in their communities by supporting them in the use of community resources, encouraging them in establishing memberships within their communities, and reducing stigma within the community through education. Provide side by side interventions to consumers that focus on problem solving, skills training, modeling behavior, optimism and encouragement. Assist consumers with working towards achieving goals and developing skills in order to help consumers be able to move into a more independent setting.
