Key Responsibilities Holistic Care Coordination: Support and monitor both the complex psychiatric and chronic medical needs of clients living within our residential group homes. Interdisciplinary Collaboration: Coordinate care directly with psychiatrists, primary care physicians, specialists, and community pharmacies to streamline client treatment plans. Medication Reconciliation: Perform thorough medication reconciliations during client transitions, admission to the program, or following discharge from external medical facilities. MAR Management: Systematically manage, update, and validate the Medication Administration Record (MAR) books across the residential program to ensure safety and clinical accuracy. Clinical Advocacy: Act as a medical resource for residential staff, providing guidance on health monitoring, potential medication side effects, and chronic disease management.
