• What You’ll Do
• Travel across the metro to support different territories and care teams.
• Provide skilled nursing visits, assessments, and care planning.
• Complete OASIS visits and coordinate patient care.
• Build trust quickly with patients and families.
• Help maintain consistent care during growth or staffing changes.

• What We’re Looking For
• Active Minnesota Registered Nurse (RN) license.
• Comfortable working independently in a mobile, field-based role.
• Strong clinical judgment and adaptability.
• Home health experience preferred, but not required.
• Reliable transportation and willingness to travel.
