Position Summary

Responsibilities

Provide patient care according to client’s care plan and needs Perform wound care treatments and assist clients with medication management Teach the client and/or family members about care and support skills Communicate regularly with RN Case Managers, Supervisors, and the Care Team

Qualifications

Licensed as a Practical Nurse / LPN in the state of Minnesota Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License Demonstrated ability to work autonomously Prior Nursing experience in a TCU, Hospice, or Home Health setting Experience with Electronic Medical Records (EMR) software is preferred Military Occupational Classification (MOC) Codes: If you're a military member transitioning to civilian life, we value your service and skills.
