Salary
$30 - $38 / HOUR
Provide direct patient care according to care plans, including wound treatments and medication management. Educate clients and families on support skills while coordinating with RN Case Managers and the care team.
Must be a Licensed Practical Nurse (LPN) in Minnesota with a valid driver's license and reliable vehicle. Experience in TCU, Hospice, or Home Health settings and proficiency with EMR software is preferred.
