Evaluate patient functional status and implement individualized care plans to improve mobility and independence in home settings. Collaborate with interdisciplinary teams and supervise Physical Therapist Assistants to ensure coordinated patient recovery.
Requires a graduate degree in Physical Therapy (DPT or Master's) and a current state PT license. Candidates should have 1-2 years of experience, CPR certification, and a valid driver's license with reliable transportation.
