Summary/Objective: Provide clinical triage, assessment, and guidance to patients via phone, patient portal, and in-person encounters. Deliver patient education regarding conditions, procedures, and recovery plans while ensuring a positive patient experience. Coordinate care with providers, advanced practice providers (APPs), interdisciplinary teams, and external facilities to support safe, timely, and high-quality patient care. Conduct postoperative follow-up phone calls to patients within a few days after discharge. Including but not limited to symptom monitoring, wound assessments, pain assessments. Reporting concerns to providers as appropriate. Maintain accurate, timely, and thorough documentation in the electronic health record. Demonstrate flexibility with work assignments and assist other members of the Care Team as appropriate. Maintain professional licensure and certifications in compliance with regulatory requirements.

Functions: Respond to patient inquiries via phone, portal, email, and in-person encounters. Ensuring timely, courteous, and clinically appropriate guidance. Perform clinical assessments. Including triage, symptom evaluation, and identifying urgent or emergent situations. Escalating to providers as appropriate. Conduct postoperative patient care and follow-up. This may include evaluating recovery progress and providing education on self-care. Implement orders based on standing protocols and provider guidance. Coordinate care with internal teams, providers, and external facilities. Including prior authorizations, referrals, and medication requests. Participate in quality improvement initiatives, including protocol and workflow development. Support immunization and other preventive care initiatives as assigned. Assist in compliance with regulatory requirements (OSHA, Department of Health, DEA, etc.) and ensure accurate clinical documentation. Utilize EHR and other essential technology to perform daily patient care and documentation tasks.

Travel
Travel to the Central Business Office or clinic locations is required to support team collaboration, training, or clinic coverage. Care team RNs may be required to work onsite approximately 1 day every other week at a clinic location, with additional onsite days as determined by leadership. Travel schedules may be adjusted to meet patient care needs, staffing coverage, or operational requirements. Care team RNs are expected to maintain flexibility and coordination with leadership when travel is needed.
