About the role

Coordinates and oversees interdisciplinary plans of care for patients within a home care environment. Collaborates with nurses, physicians, and caregivers to ensure quality and cost-effective care delivery.

What You Will Do

Works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care environment. Assesses plans, facilitates and evaluates timely coordination of quality, cost-effective care and works flexible hours and has 24-hour accountability as necessary to meet patient/family care needs.

What you will need

  • Bachelor's Degree in Physical Therapy or higher. State of WI Physical Therapist licensure. Valid State of WI Driver's License with acceptable driving record. Current BLS (CPR) certification or ability to obtain upon hire. At least 6 months experience in a home health setting preferred. At least 1 year physical therapy experience preferred.

Requirements

Requires a Bachelor's degree in Physical Therapy, WI state licensure, a valid driver's license, and BLS certification. Experience in home health and general physical therapy is preferred.
