The RN will evaluate patient health, create and implement individualized care plans, and monitor progress to maintain high standards of care. Responsibilities include administering treatments and supervising Licensed Vocational Nurses while coordinating with an interdisciplinary team.
Candidates must hold a current California Registered Nurse (RN) license and have at least one year of experience in home health or hospice care. A valid CA driver's license and the ability to travel within a 25-mile radius are required.
Provide nursing care within the scope of practice, following the patient’s individualized care plan, in coordination with the Interdisciplinary Team. Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing. Maintain accurate clinical records and submit documentation according to company policy using a company-provided device. Supervise Licensed Vocational Nurses (LVNs) when assigned. Comply with all company policies and procedures to ensure high standards of care.
