Summary of Performance Expectations

• Provides counseling and guidance support to patients and families throughout the hospital as needed.
• Assess and evaluate discharge planning needs in collaboration with physicians, direct care nurses, Patient Care Managers/Discharge Planners/Social Worker, and associated clinical professionals.
• Participates in interprofessional team meetings.
• Assists patients and families in obtaining financial assistance, psychosocial support, community resources, advance directive education, and medical equipment.
• Works closely with community agencies that provide services and resources to patients and families.
• When applicable, facilitates support groups and collects data required for accreditation programs.

Education & Credentials

Required

Master’s degree in Human Service Field (Counseling, Sociology, Psychology). May consider a Master’s degree with five or more years of experience with mental health counseling.

Required

Experience in acute care discharge/transition of care planning associated with social needs identified based on social assessment. Given training and on-the-job experience, incumbent should be proficient in the basic aspects of the job within three months of employment date.
