Provides counseling and guidance to patients and families while coordinating discharge planning with clinical professionals. Assists patients in accessing financial aid, psychosocial support, and community resources.
Requires a Master's degree in a Human Service field such as Counseling, Sociology, or Psychology. Experience in acute care discharge and transition of care planning is required.
Master’s degree in Human Service Field (Counseling, Sociology, Psychology). May consider a Master’s degree with five or more years of experience with mental health counseling.
Experience in acute care discharge/transition of care planning associated with social needs identified based on social assessment. Given training and on-the-job experience, incumbent should be proficient in the basic aspects of the job within three months of employment date.
