Are you an experienced and highly autonomous Nurse Practitioner (NP) or Physician Assistant (PA) looking for a unique opportunity to provide meaningful, high-level clinical support to seniors while enjoying an exceptional work-life balance? Seniority Healthcare is seeking a dedicated Advanced Practice Clinician to serve in a combined On-Call and Provider of the Day role supporting our geriatric population and clinical teams.
This fully remote position follows a 7 days on / 7 days off schedule, offering flexibility and extended time off while allowing you to make a substantial impact across our organization.
This unique clinical support role combines Provider of the Day responsibilities with after-hours On-Call coverage for a high-risk geriatric population.
During your scheduled week on, you will serve as the central clinical support provider for the organization, assisting field providers, addressing urgent patient concerns, conducting telemedicine visits, managing remote clinical needs, and ensuring continuity of care across the practice.
Telemedicine care is a core component of this role and includes both scheduled and urgent same-day patient evaluations. Unlike a traditional outpatient role, this position does not include management of a primary patient panel or routine clinic responsibilities.
This role is ideal for a confident clinician who enjoys problem-solving, acute clinical decision-making, virtual care delivery, and collaborating with providers and staff in a fast-paced environment.
During your scheduled week on, you will also provide after-hours clinical coverage for our patient population.
All patient calls are first screened through our answering service, which triages and forwards clinically appropriate concerns to the on-call provider.

Must be a licensed Nurse Practitioner or Physician Assistant with active licensure in PA, NY, NJ, and DE. Experience in geriatrics, family medicine, internal medicine, or emergency medicine is preferred.
