The therapist provides weekly home-based family therapy using the FFT model for families with adolescents. Key duties include performing psychosocial assessments, developing treatment plans, and conducting behavioral health interventions.
Requires a Bachelor's degree in a related field (Master's preferred) and at least two years of experience in Behavioral Health. Must hold a valid professional license such as LMSW, LISW, LMHC, or LMFT.
Overview The therapist will provide, at a minimum, weekly one-hour, home-based family therapy sessions to clients. Therapist will utilize the FFT model with families who have adolescents between the ages of 10 and 18. Therapist will be required to carry a caseload of 10 to 15 families, providing services anywhere from eight to 26 weeks. This position requires flexibility in hours to meet families when they are available.
Key Accountability: Performs comprehensive psychosocial and behavioral assessments, treatment plans and appropriate interventions and therapy for behavioral health illnesses and concerns. Conducts assessment, diagnosis and treatment of the social and emotional factors related to the patients illness, need for care, response and adjustment to illness and care and completes appropriate patient-centered plan of care and interventions according to the scope of practice. Provides information and referral services to appropriate individuals and agencies as needs identified. Conducts immediate intervention including supportive counseling to patient/family/significant other in dealing with impact of an emergency situation. Completes all documentation in the patient record accurately and timely. Provide educational in-services to the community as requested. Consistently utilizes a holistic approach considering psych/social, physical, spiritual, educational, safety and related criteria approach to the age of the patient. Participates in the development of the plan of care and attends department team meetings. Maintains current licensure with specific board. Completes all continuing education and follows Ethics Code specific to the discipline.
Education: Bachelor’s degree in social work, Human Services, Psychology or related studies Required. Master’s degree in social work, Counseling or a related field from an accredited school preferred.
