The Outreach LPN is responsible for patient rooming, communicating medical results to patients and providers, and assisting with clinic procedures. They also ensure the efficient completion of day-to-day clinic activities.
Candidates must be a graduate of an accredited nursing school and hold a current LPN license in the State of Minnesota. A current American Heart Association BLS Healthcare Provider card is also required.

The LPN will provide patient rooming for all types of clinic visits, communicate results and other pertinent medical information to patients and providers, assist in assuring the day-to-day activities of the clinic are completed efficiently and assist providers with clinic procedures among various other duties.
To be eligible for the hiring and relocation bonus, you must be an external candidate who is hired into an eligible full-time or part-time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire. Relocation assistance also requires that you currently reside more than 50 miles away and relocate to within 30 miles of your primary work location within 120 days.
