The RN Case Manager is responsible for coordinating patient care and managing cases within the CHI St. Alexius Health network. This role involves ensuring high-quality care delivery and supporting the health system's legacy of innovation.
The candidate must be a Registered Nurse capable of working a day shift including rotating weekends and holidays. The role requires the ability to work on-site in Bismarck, ND.
Conduct comprehensive assessments of patients' medical, social, psychological, and financial needs to identify barriers to discharge and develop individualized care transition plans. Coordinate and facilitate the delivery of appropriate healthcare services, collaborating with physicians, nurses, social workers, insurance providers, and community resources to ensure continuity and quality of care. Advocate for patients and their families, ensuring their rights are respected, their preferences are considered, and they are empowered to make informed decisions about their healthcare journey. Manage patient caseloads, monitoring progress, evaluating the effectiveness of interventions, and making necessary adjustments to care plans to achieve optimal patient outcomes and efficient resource utilization. Educate patients and families on their health conditions, treatment options, available resources, and discharge instructions, preparing them for successful transitions to home, rehabilitation, or other care settings. Maintain accurate and timely documentation of all case management activities, assessments, interventions, and communications, ensuring compliance with hospital policies, regulatory requirements, and payor guidelines. & THIS JOB OFFERS BOTH AN INCENTIVE AND RELOCATION: We are offering a $5,000 Sign-On Bonus in addition to relocation to new hires who meet the eligibility requirements. Union Work Agreement: FTE Status: Full Time - 1.0; 80 hours per pay period Shift Type/Length: Days, 8 hours On Call Expectations: None Weekend/Holiday Frequency: Rotating Weekends & Holidays Job Requirements Required Qualifications Nursing Degree from an approved school of nursing LICENSURE Active Registered Nurse license in good standing with the North Dakota Board of Nursing or Active Registered Nurse license in a compact state Working knowledge of patient care across various healthcare settings, care coordination principles, and utilization review Basic Life Support (BLS) Certification within the orientation period and meets re-certification as required Good communication skills necessary for constant communication, both oral and written, with patients, families, visitors, and medical center staff Accountable for own conduct and promotes and supports positive working relationships among staff, between other disciplines, departments, families, visitors, and patients Maintains confidentiality
