Promotes patient and family wellness by managing comprehensive transitional care plans for patients with complex psychosocial needs. Collaborates with physicians, external agencies, and service teams to ensure quality care and effective transitions in Inpatient and Emergency Department environments.
Requires a Bachelor's Degree in Social Work from an accredited program and at least one year of experience in an acute or post-acute setting. Professional certifications such as ACM or CCM are preferred.
This position strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Works to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.
