The Nurse Practitioner assesses, diagnoses, and implements medical care plans for patients in collaboration with physicians. They are responsible for coordinating holistic care, providing patient education, and managing transitions of care within an interdisciplinary team.
Requires a Master's Degree in Nursing and valid California NP, RN, and Furnishing licenses. Ideally, the candidate should have at least 5 years of recent relevant clinical experience and BLS certification.
ASSESSMENT AND PLANNING OF PATIENT CARE Conducts comprehensive patient and family assessments, including physiological, emotional, and spiritual needs. Collaborates with physicians and interdisciplinary team members to develop and adjust patient-centered care plans. Communicates effectively to ensure continuity and coordination of care. Partners with peers, providers, and leadership to improve patient outcomes and care quality. Supports workflow optimization to enhance patient care delivery. Communicates care plans across teams and departments. Facilitates patient transitions to community providers when appropriate. PROVISION OF PATIENT CARE Delivers high-quality, compassionate, and cost-effective care aligned with organizational policies and evidence-based practices. Maintains a safe, therapeutic environment for patients and their families. Delegates tasks appropriately based on team members’ scope, licensure, and competency. Provides clear, patient-centered education using teach-back methods to ensure understanding. Practices mindful, individualized care with a strong focus on patient safety. Ensures compliance with all organizational, local, state, and federal regulations. Safeguards patient confidentiality and protects all protected health information (PHI). Stays current with advancements, best practices, and trends in clinical care and specialty areas. Participates in and supports process improvement initiatives at all organizational levels. EVALUATION OF PATIENT CARE Monitors interventions, evaluates patient responses, and adjusts care plans accordingly. Provides ongoing patient and family education throughout the continuum of care. Reassesses and documents patient progress and outcomes. Reports safety events or near-misses through proper reporting systems. Participates in the analysis of safety events to prevent recurrence. Maintains accurate and timely documentation in the electronic health record (EHR). Develops and implements comprehensive discharge and transition education plans involving patients and families.
