• Purpose: - The Case Manager I at Behavioral Health Services Inc is responsible for providing comprehensive case management services to clients with behavioral health needs. The role involves coordinating care, advocating for clients, and ensuring access to necessary resources and services to promote their overall well-being and recovery.
• Key
• Responsibilities: - Conduct thorough assessments of clients' needs, strengths, and challenges to develop individualized care plans. - Coordinate and monitor the delivery of services, ensuring that clients receive appropriate interventions and support. - Advocate for clients' needs and rights within the healthcare system and community resources. - Facilitate access to medical, psychological, and social services by collaborating with healthcare providers, community organizations, and other stakeholders. - Maintain accurate and up-to-date documentation of client interactions, progress, and outcomes in compliance with organizational and regulatory standards. - Provide crisis intervention and support to clients as needed, ensuring their safety and stability. - Educate clients and their families about available resources and support systems to empower them in their recovery journey. - Participate in multidisciplinary team meetings to review client progress and adjust care plans as necessary. - Stay informed about current trends and developments in behavioral health to enhance service delivery and client outcomes.
• QualificationsRequired
• Education: - Bachelor's degree in Social Work, Psychology, Counseling, or a related field from an accredited institution.
• Required
• Experience: - Minimum of 1 year of experience in case management within a behavioral health setting. - Experience working with diverse populations and understanding of cultural competence in service delivery. - Prior experience in developing and implementing individualized care plans.
