The Step-Down Navigator facilitates the transition of patients from inpatient psychiatric settings to PHP/IOP levels of care. They act as an intermediary between hospital departments and families to eliminate barriers and improve outpatient conversion rates.
A high school diploma or GED is required, while a bachelor's degree and two years of psychiatric healthcare experience are preferred. Candidates must obtain certifications in CPR, First Aid, Handle with Care, and Verbal De-Escalation upon hire.
The Step-Down Navigator works with patients and their families to successfully transfer discharging patients from the inpatient setting into the PHP/IOP level of care. Focus will be on improving step down conversion rates and contributing to the increase and stabilization of the outpatient census. The Step-Down Navigator works directly with various members of the healthcare team, hospital departments, and administration to answer questions, promote the program and eliminate barriers to successful step downs in level of care with the goal of facilitating a positive patient experience during their time in the organization. The Step-Down Navigator acts as an intermediary between hospital departments and the patient/family to bring about the most effective healthcare treatment and positive hospital experience while promoting the outpatient program and navigating barriers to successful step downs.
High school diploma/GED required, BA preferred. Preferred two (2) years experience in a psychiatric health care setting.
Must obtain CPR, First Aid, Handle with Care, and Verbal De-Escalation certifications provided by facility upon hire.
