Hospice/Home Care Register Nurse

Responsibilities

• Provide skilled nursing care to hospice and home care patients according to established plans.
• Assess patient condition and effectively communicate changes to the care team.
• Develop, implement, and update individualized care plans in collaboration with interdisciplinary teams.
• Educate patients and family members regarding illness, treatment options, and self-care techniques.
• Administer medications and treatments as prescribed by physicians.
• Document all patient interactions and care activities accurately and timely.
• Coordinate with physicians, social workers, and other healthcare professionals to provide holistic care.

Requirements

• Current Registered Nurse (RN) license in the state of Michigan.
• Experience in hospice or home care nursing preferred.
• Strong assessment, communication, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Valid driver’s license and reliable transportation.
