Remote RN Case Manager (Costco Team) Location: Remote (Preferred: within 45 minutes of San Antonio, TX) Schedule: Monday–Friday, 8:00 AM–5:00 PM CST (occasional rotation to 10:00 AM–7:00 PM) No weekends or holidays
Join a dedicated care team supporting Costco members as a Remote RN Case Manager. In this role, you’ll make a meaningful impact by guiding members through their healthcare journey—assessing needs, coordinating care, and improving overall wellness outcomes. What You’ll Do Conduct telephonic (and occasional in-person) member assessments Develop and manage personalized care plans to improve health outcomes Identify and address clinical, social, and functional barriers to care Coordinate resources, referrals, and transitions across care programs Collaborate with interdisciplinary teams to support complex cases Engage members through effective communication to drive positive results What You Bring - REQUIRED Active, unrestricted multistate RN license (or willingness to obtain additional licenses—company paid) 3+ years of clinical RN experience Comfort navigating multiple systems (Microsoft Office, web-based tools) Strong critical thinking, communication, and member engagement skills Associates Degree in Nursing Bonus Qualifications Case management experience in an integrated care model Bilingual (Spanish/English) BSN preferred Additional Details Preferred residence within driving distance of San Antonio office (occasional onsite meetings) Associate’s Degree in Nursing required Why Join Us? The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
