Acts as the primary point of contact for the Adult Recovery Team to coordinate patient-centered care and facilitate collaborative team meetings. Develops personalized service plans and connects patients with community resources to support behavioral health and wellness goals.
Requires a high school diploma with specific behavioral health education and experience, or four years of full-time behavioral work experience. A bachelor's or associate degree with relevant experience is preferred, along with an Arizona Fingerprint Clearance Card.
