Objectives
Carry a full, high-quality caseload: ramp to ~28 productivity points/week at target visit quality.
Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
What you'll own
Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits. Complete comprehensive in-home assessments and deliver skilled PT visits.
Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving. Manage treatment and outcomes directly, including PTA supervision where appropriate. Coach patients and caregivers with practical home programs that actually get done. Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Full-time target: ~28 productivity points/week; clinicians earn more for exceeding it.
Guaranteed floor: ~24 points/week for the first 90 days - if your caseload hasn't filled, you're still paid the floor.
