Working under the supervision of a Registered Nurse, you will: Deliver high-quality skilled nursing care according to physician orders and individualized care plans Monitor patient conditions and identify changes requiring intervention Provide wound care, medication management, disease education, and other skilled nursing services Educate patients and caregivers on self-care techniques, treatment plans, and safety measures Collaborate closely with RN Case Managers, physicians, therapists, and other members of the care team Document patient visits accurately and timely in the electronic medical record Assist in implementing care plans designed to improve outcomes and reduce avoidable hospitalizations Communicate significant findings and patient concerns to the interdisciplinary team Promote patient independence, safety, and quality of life in the home setting Schedule & Coverage Full-time position Coverage area: Cumberland-Portland region Rotating on-call schedule approximately once every 8 weeks Flexible scheduling with support from local clinical leadership What You'll Need Graduate of an accredited Practical Nursing program Current, unrestricted Licensed Practical Nurse (LPN) license Minimum of one (1) year of nursing experience in a clinical setting Valid driver's license, reliable transportation, and current auto insurance Ability to travel throughout the assigned service area Strong communication, assessment, and organizational skills Physical Requirements: Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
