Coordinate social services for patients at Charter House, focusing on patient needs and collaboration with other care providers. Manage information collection and prioritization to support decision-making processes in a healthcare setting.
A Bachelor's degree in Social Services or Social Work is preferred, or an LPN qualification. Experience in long-term care, post-acute rehabilitation, or home health is highly desired, along with a preference for LSW licensure.
A Bachelor's degree from an accredited college or university in Social Services or Social Work preferred. Or graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
Must be able to work in collaboration with others. Is knowledgeable about Medicare and other third-party payors. Requires good communication and human relation skills, strong problem-solving skills are necessary with the ability to collect, compare, sort and prioritize information to be used in the decision-making process. Must be able to adapt to unpredictable situations.
