Promotes patient and family wellness by managing comprehensive transitional care plans for patients with complex psychosocial needs. Collaborates with physicians, external agencies, and service team members to ensure effective transitions from inpatient and emergency department environments.
Requires a Master's degree in Social Work from an accredited program and a state license as a Licensed Medical Social Worker (LMSW). Certification as an Accredited Case Manager (ACM) or Certified Case Manager (CCM) is preferred.
This position strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Works to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.
