Candidates must hold a Community Health Worker Certification from a Connecticut-approved provider. Required skills include proficiency in crisis management, computer literacy, and knowledge of regional behavioral health resources.
The SCY Community Health Worker (CHW) provides outreach, screening, advocacy, and barrier intervention for families of children referred to the Screening Connecticut’s Youth program for identification of at-risk mental health indicators and referral to behavioral health interventions. The CHW ensures the coordinated access to community-based supports and behavioral health treatment resources to meet identified client needs.
